Tuition Agreement Form

I agree to make automatic payments to Vine & Branches Child Development Center via Brightwheel for the rate indicated above on a weekly basis and understand that my payment will be automatically withdrawn every Wednesday for the upcoming week. I understand that I must create a Brightwheel account, enroll in autopay, and add my bank account information. Additionally, I understand that if I need to cancel or change my payment method, I am responsible for doing so in the Brightwheel system, or by contacting Brightwheel directly. I understand that I am responsible for paying a 2.9% transaction fee for credit card payments or a flat $.60 transaction fee for ACH payments. I furthermore permit Vine & Branches to bill the Registration Fee to my account, and any other fees deemed necessary per the parent handbook schedule of fees (i.e. late fees, late pick up fees, etc.). This authorization is to remain in effect until all amounts payable are paid in full or until I revoke the agreement in writing.
