Register Your Child
To register your child, we need these three forms: Your Child’s Registration Form, the School Entrance Health Form (Signed by and available from your Doc), and the Tuition Agreement Policy.
Additional Forms
We may also need additional forms once your child is enrolled, should you choose to utilize our vacation credit, should your child need over-the-counter skin products or medications, or if you are enrolling an infant with us. Please inquire with our friendly office staff should you have any questions about which forms you need to submit.
